Visa Fees

Basic visa fee is EUR 80. Children aged six to under 12 pay a visa fee of EUR 40.
** The fees must be paid at the office counter when submitting the application. 
**  Visa fee is payable in cash. 
** Please note that this is non-refundable, whatever the consul's decision.

Applicant:

  • Personal Appearance
  • Application Form
  • Original NSO Authenticated Birth Certificate and/or Marriage Certificate
  • Passport with no less than 3 months validity over the visa expiration and/or Previous Passport
  • 3 pcs. passport-sized photo on a white background with collared shirt (with your name, date of birth and signature at the back of the photo)
  • Original or Notarized copy of Proof of Funds – Original Bank Certificates and Bank Statements (e. g. Passbook, time deposit certificates, history of ATM account., credit card statements – history of transactions must be for the past 6 months prior from the date of submission to the embassy)
  • Original or Notarized copy of Proof of Assets (e. g. land titles, condos, deed of sale, tax declaration, deed of donation)
  • Photocopy of LTO Car Registration and Official Receipts
  • Documents indicating the purpose of the journey and countries to be visited
  • e.g. the detailed day to day itinerary of the planned trip, confirmation of the order travel from host organization (voucher).

  • Travel Insurance
    • The sum insured has to exceed 30.000 Euro.
    • Only a policy issued by an insurance company based in Europe or listed insurance companies are accepted by the Embassies of the Schengen Countries in Manila.
    • The policy shall be valid for all Schengen area states.
    • The policy shall cover at least the entire period of the first trip.
  • Proof of Relationship-Pictures with the Sponsor, printed chats and emails, Stamp pages of passports, Letters, Cards, Hotel Receipts, Restaurant Receipts, itineraries, Money Remittances, Airway bills




Employed:
  • Original Employment Certificate – stating the length of service, monthly salary and position
  • The letter must include the name and position of the person who signed the certificate, registration and contact details of the company.
  • Approved Leave of Absence
  • Scan copy of Company ID, Payslips (recent 6 months), ITR's





Self-Employed:
  • Proof of Ownership – Company Business Registration Certificates, DTI/ SEC Permit, Mayor's Permit, Sanitary Permit and ITR's for past three (3) years,





Student:

  • Original Certificate from the School / University
  • Other School Credentials (e. g. Study Load) and School ID





Sponsor(s):


  • Proof of Sponsorship and/or private accommodation (please note: the signature on the form must be legalised by the municipality)
  • Original Invitation Letter – stating the length of stay, reason of the visit, must have complete names, addresses and contact numbers of the invitee and inviter
  • Notarized Original Affidavit of Support
  • Proof of Relationship – Scan copy of Birth and Marriage Certificate
  • Proof of Citizenship – Scan copy of Permanent Residency Card and Passport with stamp pages
  • Proof of Accommodation – Proof of Ownership of House, Car and Car Insurance (Deed of Registry), Tenancy Agreement/Letter from the Landlord
  • Proof of Funds – Bank Certificate, Bank Statement (history of transactions must be for the past 6 months prior from the date of submission to the embassy) Credit Card Statements, Notice of Assessments






Employed:

Flour types in Poland

Flour type, according to Polish food regulations, means the content of ashes in it (i.e. the remains after complete burning of the organic ingredients in a sample of the product at a determined temperature). Police clearance certificate in Poland
