INFO SHEET ON NBI CLEARANCE CERTIFICATE


A. NBI Clearance for those with NBI Clearance from 2014 onwards

New applicants who were previously issued NBI Clearance Certificates from 2014 onwards can renew their NBI Clearance Certificate directly with the NBI without the need for a new fingerprint card (NBI Form No. 5), provided no changes are made on their personal data.

There is no need to accomplish NBI Form No. 5 and affix fingerprints taken at the Embassy if your old NBI Clearance Certificate was issued from 2014 to the present, and no correction was made in your name, place and date of birth (for example, a female who marries and changes to her married surname).

Send the following requirements to your authorized representative in the Philippines who will file the application on your behalf with the NBI:
  1. Authorization letter;
  2. Old NBI Clearance Certificate;
  3. 2x2 photo taken within the last three (3) months;
  4. Photocopy of your valid passport; and
  5. Fee of PHP 200.00

B. NBI Clearance for those with NBI Clearance before 2014, changed names or first time applicants


For those with old NBI Clearances before 2014, or have changed their names since their last NBI Clearance (for example, for females after they marry), or have not applied for NBI Clearance before:
  1. Accomplish NBI Clearance Fingerprint Card (NBI Form No. 5) at the Embassy. The Embassy staff will supervise and assist you in taking your fingerprints. Submit also a recent 2x2 ID photograph with white background taken within the last 3 months.
  2. Pay the attestation fee PLN 100,00 at the Embassy.
  3. After fingerprinting, the Embassy will give you the NBI Clearance Fingerprint Card, attested and with your fingerprints.
Send the following requirements to your authorized representative in the Philippines who will file the application on your behalf with the NBI:
  1. Authorization letter;
  2. Old NBI Clearance Certificate;
  3. NBI Clearance Fingerprint Card with 2x2 photo taken within the last three (3) months;
  4. Photocopy of your valid passport; and

Police clearance certificate in Poland

The certificate is issued by the Information State Bureau of Criminal Register of the Ministry of Justice of Poland. The certificated may be apostilled by the Ministry of Foreign Affairs of Poland.
