The following documents are needed by the Polish Embassy for processing of wor visa:
- Original passport and visa fee
- Registered application form SIGNED BY APPLICANT in both places with date, with 1 passport size colour photograph (white background)
- 1 passport size colour photograph (white background)
- Copy of passport biopage only (page with photo)
- Copy of ID card
- Work Permit
- Preliminary employment agreement
- Written declaration from employment
- Proof of accommodation
- Documents proving the scope of activities of the employer
- Health Insurance
- Confirmed flight ticket
- Work experience certificate
- Copies of educational certificates
- CV – curriculum vitae
- Confirmation of sufficient financial means
